Click on the Segment I Contract link below. You can fill in the blanks that the contract lets you fill in and leave empty the other blanks it won't let you fill in. Or you can simply print it out and fill it in by hand. Print a second copy for your records and have your student bring in the other copy signed by parent or guardian.
No student will be allowed to drive until a signed Segment I Contract has been turned in to the class instructor!
